Uploading Form 15H – Taxation – beragampengetahuan – Beragampengetahuan

I hold some listed bonds and has been emailed to upload 15H form before 31 January to avoid TDS deduction.

The webpage mentions “Duly filled and certified Form 15G/15H.”
What does certified in above context means? Do I need to get it attested by a gazetted officer?
Any information on the process or documentation will be helpful.
